A maximum of 40 hours of PTO time may be cashed in each year. PTO time will not be paid upon termination of employment.
The submission of this form is a formal request for PTO, but does not guarantee automatic approval. Check the approval status of this request with your supervisor.
* Only for Foremen: Foreman taking PTO must assign a responsible person who will fill in for you while on PTO. The Responsible person must be competent and have the appropriate work orders, paperwork, information to properly do job.
